You're in the Right Place If...
Whether you're caring for someone, living with changes, or wanting to support your community, there's a path forward.
You're a Care Partner
You’re exhausted and knee-deep in the hard moments. Or maybe you’re doing alright for now, but curious about what comes next and wanting support before you need it. Wherever you are in this journey, you deserve guidance, reassurance, and someone who truly understands.
You want practical tools and emotional support to navigate this journey without burning out and you need someone who truly understands.
You're Living with Dementia
You've noticed changes: words that won't come, moments of confusion, things that aren't where they should be. You want to understand what's happening and how to move forward with dignity.
You're still you, and you want to keep living fully, with support that honors who you are and what remains possible.
You're a Community Member
You work with the public: in a bank, grocery store, salon, or as a first responder and you want to better support people experiencing brain changes.
Or perhaps you're a neighbor, friend, or business owner who wants to create a more inclusive, dementia-friendly community where everyone feels welcome and supported.
Working with What Remains
"When you know one person with dementia, you know one person with dementia."
Everyone is unique and so is their journey. There's no one-size-fits-all approach here, because no two experiences are the same.
I don't focus on what's lost. Together, we'll discover what's still possible, one moment at a time. Using Teepa Snow's Positive Approach to Care® , my training as an End of Life Doula and my lived experience, I help families and communities shift from frustration to connection, from fear to understanding.
This isn't about fixing or changing anyone. It's about meeting people where they are, honoring what remains, and creating meaningful moments along the way. Whether you're a care partner looking for relief, someone newly diagnosed seeking clarity, or a community member wanting to help: I'm here as your calm in the storm, a soft place to land.

Every Moment Matters: Dementia Transition Care Plan
Because every moment deserves compassion, connection, and care.
Every Moment Matters: Dementia Transition Care Plan is a compassionate framework designed to bring peace, clarity, and comfort during the final stages of dementia. This plan is meant to be created ahead of time, before your loved one journeys too far down the road of dementia — allowing families to stay fully present when it matters most.
Expanding upon traditional tools like an Advance Directive and Will, this plan gently explores and documents the wishes, needs, and personal comforts of the individual journeying through their final months, weeks, and days — right through to their final breath. It offers clear guidance for those at the bedside, outlining what brings comfort, who they wish to be present, and what is not wanted.
Rooted in empathy, dignity, and understanding, the Dementia Transition Care Plan also creates space for anticipatory grief — helping loved ones process emotions, honour memories, and find peace in the sacred transition of life.
Grief Support Groups
Gentle, guided support for both anticipatory grief and bereavement. Loving someone with dementia involves many losses along the way, and you don't have to carry that weight alone. These groups provide a safe space to share, process, and find connection with others who understand what you're experiencing.
Community Training
Custom workshops designed for businesses, care facilities, first responders, and community organizations. I help create dementia-friendly spaces where people experiencing brain changes feel welcomed, understood, and supported. Training includes practical techniques for real-world situations and can be tailored to your specific needs.
What People Are Saying
I didn't know that was even possible. The tools Heather shared completely changed how I approach my mom's care. For the first time in months, I feel like I have hope again. When's your next class? I'd love to take it.

We are visiting a friend of ours and ask him if he remembers certain times, - sometimes he does, sometimes he doesn’t, so thank-you so much for sharing your information with us. That’s really helpful and we’re going to try that!
